Driver Education Provider Certificates of Completion (DEE-964, ADEE-1317, and Public School DE-964E)

Driver Education Certificates

Driver Education Providers and Public Schools offering the Teen Driver Education Course and/or the Exclusively for Adult 6-hour course must create a certificate of completion using the TDLR model DEE-964 and or ADEE-1317 template. TDLR will send the templates to an approved provider during the initial application process.


Issuing Certificates to Students

Schools may only issue certificates to students who have successfully completed all elements of an approved driver education course.

Providers for the Teen course may issue the “FOR LEARNER LICENSE” certificate once the student has successfully completed Module 1, or the first 6 hours of the course.

Providers must fill out all required fields before they can issue a certificate to a student. The certificate must bear a control number on the top right-hand corner of the certificate.  No students should receive the same control number.  The information in each field may be typed or handwritten in black ink. 

Providers must issue the certificates to the student upon successful completion of each phase and course.

The DPS copy of a driver education certificate must contain the original signature of the driver education instructor, or the designated parent-taught driver education instructor, as applicable. A digital certificate is acceptable. The name of the driver education provider owner or its designee may be written, stamped, or typed.

Driver Education Providers and Public Schools must refer to the certificate specifications created for the applicable certificate and use them when creating a certificate.

When issuing certificates to students, providers must develop and maintain effective policies and processes to ensure the constant privacy, security, and integrity of all personal and financial confidential student information and make the privacy policy available to all students.


Securing Access

Providers and Public Schools must maintain effective protective measures to ensure the security of driver education certificate numbers and prevent the unauthorized production or misuse of the certificates, as well as ensuring the recovery of lost data (electronic or otherwise) of certificate numbers.


Student Transfers

A student enrolled in an in-person, online, or parent-taught course may transfer to another licensed driver education provider. (An enrolled student transferring to another course with the same provider is not considered a transfer.) Reporting to TDLR

EFFECTIVE DATE TO BE DETERMINED

Driver Education Providers are required to upload certificate completion data to the department within 15 calendar days upon completion of each phase, including data for ADE-1317s and DE-964s issued to a student.

Providers will only submit data electronically through the Online Licensing Services System. No faxes, emails, or paper copies will be accepted.

 The department will provide more information, including a Training Guide, for this process when it becomes available.
